Switch from failure to anyhow.

  - Use the anyhow crate instead of failure to implement the dynamic
    side of our error handling.  anyhow::Error derefs to dyn
    std::error::Error, allowing better interoperability with other
    stdlib-based error handling libraries.

  - Fixes #444.
