The retured value from ib_dma_map_sg saved in dma_nents variable. To avoid
future mismatch between types, define dma_nents as an integer instead of
unsigned.

Keeping single line wrapper functions is not useful. Hence remove the
ib_sg_dma_address() and ib_sg_dma_len() functions. This patch does not
change any functionality.

ib_dma_map_sg() augments the SGL into a 'dma mapped SGL'. This process may
change the number of entries and the lengths of each entry.
Code that touches dma_address is iterating over the 'dma mapped SGL' and
must use dma_nents which returned from ib_dma_map_sg().
ib_sg_to_pages() and ib_map_mr_sg() are using dma_address so they must use
dma_nents.

Pull SCSI updates from James Bottomley:
"This is mostly update of the usual drivers: smarpqi, lpfc, qedi,

Additionally, we have a pile of annotation, unused variable and minor

The big API change is the updates for Christoph's DMA rework which

Most SCSI drivers want to enable "clustering", that is merging of
segments so that they might span more than a single page. Remove the
ENABLE_CLUSTERING define, and require drivers to explicitly set
DISABLE_CLUSTERING to disable this feature.

If for some reason we failed to query the mr status, we need to make sure
to provide sufficient information for an ambiguous error (guard error on
sector 0).

In case target remote invalidates bogus rkey and signature is not used,
pi_ctx is NULL deref.
The commit also fails the connection on bogus remote invalidation.

Since the next patch will change the return type of these functions into a
const pointer and since the iSER driver modifies the work request these
functions return a pointer two, inline two work request conversion
function calls. This patch does not change any functionality.

Currently this function is implemented in the scsi layer, but it's
actual place should be the block layer since T10-PI is a general
data integrity feature that is used in the nvme protocol as well.

We need to set can_queue earlier than when enabling the scsi host.
in a blk-mq enabled environment, the tagset allocation is taken
from can_queue which cannot be modified later. Also, pass an updated
.can_queue to iscsi_session_setup to have enough iscsi tasks allocated
in the session kfifo.

The kmalloc() function has a 2-factor argument form, kmalloc_array(). This
patch replaces cases of:
kmalloc(a * b, gfp)
with:
kmalloc_array(a * b, gfp)
as well as handling cases of:
kmalloc(a * b * c, gfp)
with:
kmalloc(array3_size(a, b, c), gfp)
as it's slightly less ugly than:
kmalloc_array(array_size(a, b), c, gfp)
This does, however, attempt to ignore constant size factors like:
kmalloc(4 * 1024, gfp)
though any constants defined via macros get caught up in the conversion.
Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.
The tools/ directory was manually excluded, since it has its own
implementation of kmalloc().

The iSER driver reduces max_sectors. For example, if you load the
ib_iser module with max_sectors=1024, you will see that
/sys/class/block/<bdev>/queue/max_hw_sectors_kb is 508. It is an
incorrect value. The expected value is (max_sectors * sector_size) /
1024 = 512.
Reducing of max_sectors can cause performance degradation due to
unnecessary splitting of IO requests.
The number of pages per MR has been fixed here, so there is no longer
any need to reduce max_sectors.

NOTE! On almost all architectures, the EPOLL* constants have the same
values as the POLL* constants do. But they keyword here is "almost".
For various bad reasons they aren't the same, and epoll() doesn't
actually work quite correctly in some cases due to this on Sparc et al.
The next patch from Al will sort out the final differences, and we
should be all done.

The ib_mr->length represents the length of the MR in bytes as per
the IBTA spec 1.3 section 11.2.10.3 (REGISTER PHYSICAL MEMORY REGION).
Currently ib_mr->length field is defined as only 32-bits field.
This might result into truncation and failed WRs of consumers who
registers more than 4GB bytes memory regions and whose WRs accessing
such MRs.
This patch makes the length 64-bit to avoid such truncation.

The functions ib_register_event_handler() and
ib_unregister_event_handler() always returned success and they can't fail.
Let's convert those functions to be void, remove redundant checks and
cleanup tons of goto statements.

All modules in drivers/infiniband defined and used MODULE_VERSION, which
was pointless because the kernel version describes their state more accurate
then those arbitrary numbers.

Under heavy iser target(scst) start/stop stress during login/logout
on iser intitiator side happened trace call provided below.
The function iscsi_iser_slave_alloc iser_conn pointer could be NULL,
due to the fact that function iscsi_iser_conn_stop can be called before
and free iser connection. Let's protect that flow by introducing global mutex.

max_fast_reg_page_list_len is only valid when the
memory management extentions are signaled by the underlying
driver.
Fix by adjusting iser_calc_scsi_params() to use
ISCSI_ISER_MAX_SG_TABLESIZE when the extentions are not indicated.

When the rdma device is removed, we must cleanup all
the rdma resources within the DEVICE_REMOVAL event
handler to let the device teardown gracefully. When
this happens with live I/O, some memory regions are
occupied. Thus, track them too and dereg all the mr's.
We are safe with mr access by iscsi_iser_cleanup_task.

Bart Van Assche noted that the ib DMA mapping code was significantly
similar enough to the core DMA mapping code that with a few changes it

Instead define the timeout behavior purely based on the host_template
eh_timed_out method and wire up the existing transport implementations
in the host templates. This also clears up the confusion that the
transport template method overrides the host template one, so some
drivers have to re-override the transport template one.

For devices that can register page list that is bigger than
USHRT_MAX, we actually take the wrong value for sg_tablesize.
E.g: for CX4 max_fast_reg_page_list_len is 65536 (bigger than USHRT_MAX)
so we set sg_tablesize to 0 by mistake. Therefore, each IO that is
bigger than 4k splitted to "< 4k" chunks that cause performance degredation.
Remove wrong sg_tablesize assignment, and use the value that was set during
address resolution handler with the needed casting.

Instead of exposing ib_get_dma_mr to ULPs and letting them use it more or
less unchecked, this moves the capability of creating a global rkey into
the RDMA core, where it can be easily audited. It also prints a warning
everytime this feature is used as well.

The SRP initiator allows to set max_sectors to a value that exceeds
the largest amount of data that can be mapped at once with an mlx4
HCA using fast registration and a page size of 4 KB. Hence modify
ib_map_mr_sg() such that it can map partial sg-elements. If an
sg-element has been mapped partially, let the caller know
which fraction has been mapped by adjusting *sg_offset.

iSER currently has a couple places that set max_sectors in either the host
template or SCSI host, and all of them get it wrong.
This patch instead uses a single assignment that (hopefully) gets it right:
the max_sectors value must be derived from the number of segments in the
FR or FMR structure, but actually be one lower than the page size multiplied
by the number of sectors, as it has to handle the case of non-aligned I/O.
Without this I get trivial to reproduce hangs when running xfstests
(on XFS) over iSER to Linux targets.
